How we collect, store, and handle your personal information at Build Disability Supports.
When you first become a client of this organisation, you and your next of kin, nominee, guardian, or support provider will be asked questions about you so that we can set up a file for you and begin providing you with support and services. We will only collect information that is necessary for us to support you.
This information includes, but is not limited to:
While you are a client, we will also record information about you that is necessary to provide you with ongoing support. Before this happens, we will ask you or your next of kin, nominee, or guardian to sign our Consent to Share Form and make sure you understand what you are signing.
We store your personal information securely. Physical copies are kept in a locked office area, and online copies are stored on a private cloud system that is only accessible by those given an access login.
This information is only accessed by staff members who are directly involved in providing you a service, including those in administrative roles within Build Disability Supports.
We store this information while you are a client. After you are no longer a client, we keep your file for five years and then destroy it.

Yes. You or your next of kin, nominee, or guardian can request the information we have on file about you, and we will make it available to you at a reasonable time. We ask that the request is made in writing and addressed to the manager.
We have a complaints policy and procedure which can be accessed on our website, along with our feedback form.
If you have a complaint about the way we deal with privacy issues, please contact us or complete the feedback form, and we will endeavour to resolve the issue.
